I heard that one of the visitors performing a JCAHO (Joint Commission on the
Accreditation of Healthcare Organizations) inspection in a hospital in
California recommended that the hospital staff keep a record of the lot
numbers of all formula given.

 

This was enough to convince that hospital to stop including formula in their
gift bags (but not the coupons <sigh>)

 

.and by the way, I HAVE seen formula looking as described - in Puerto Rico -
the transportation in container ships leads to it being stored in
temperatures much higher than those recommended, probably leading to
deterioration much closer to the expiration date.I imagine the same would be
in other tropical and poor countries.
