Hello,
I have a question for all you Wise ones!
We have a multip mom on the floor with a history of itchy sensative
nipples throughout this whole pregnancy (but not with the last two
pregnancies). Her Dr. had her try hydrocortisone, with only minimal
relief. I saw her 12 hours after delivery yesterday. Her nipples slightly
pink and have crevices around the base of both, which she said were there
BEFORE even latching baby on the first time. Obviously, very painful with
latching. She has no hx of yeast infections, no ABX during pregnancy. I
discussed APNO with her Dr. and he OK'd it. Unfortunately, I called five
area pharmacies that could compound it, and none had all the ingredients
and wouldn't get it in until next week due to the weekend. Dr. OK'd just
trying Lotrimin and Polysporin ung. We decided to try one on each nipple
to see what seemed to help. My question is, can she still use Soothies?
Mom really likes them. I know it isn't recommended to use with the
lansinoh ung, and my feeling is if it is yeast, she will keep exposing her
nipples to yeast while healing (you can reuse Soothies up to a week).
What about giving her new Soothies daily? Baby L/O well with good
swallows heard. Baby's oral anatomy appears normal, mom's technique
latching on OK-we worked a little on positioning yesterday. Nipple came
out round after. This is her third baby and with the first BF "didn't
work", mom doesn't remember why as she was young. With her second didn't
try BF. Nipples very painful, seems motivated to continue. Discussed
maybe pumping a while as we try to heal nipples, mom not sure she wants to
do this. Any other suggestions? Could the itching just be hormonal?

Thanks for any help,
Roxanne Gollata, RN, LC Intern
