Hello Lactnetters,
I had a rather interesting episode this week.
A last in the ward who had a Caesar, was unable to feed on one side!
She related how on and off this pregnancy this same thing had happened and
the Dr told her after she had the baby it would cease. She had never had it
previous pregnancies.
Now this poor  tiny lady had been spoon feeding her little 'rose
bud-mouthed' bub as it refused to fix either breast that when I was called
were like proverbial watermelons and the nipples were huge doorknobs on the
ends. 40+ " at least! The first I expressed out using Hot towels and
shoulder massage to allow her to relax and the milk literally dripped as
fast as I expressed- Relieved somewhat I fixed bub after much patience re
attaching in Football hold and wrapped around mum and it fed perfectly and
Purred!!

However the other Breast was 'strange' to touch the areola, was like
touching a very oedematous, raised well above the rest of the breast area
that crackled under your fingers. It appeared very hot and painful too! I
initially applied heat to see if the breast pressure would be relieved but
the 'dripping' didn't occur. The nipple wasn't oedematous but bub was too
full to suck at this side and a breast pump, which she had tried was too
painful. Any was it was the bulb sort which I find is the only one available
here and doesn't give suction, but  falls to bits and is useless! A big
waste of clients money!
Checking via an interpreter in case I missed something in the initial
conversation regarding an allergy or reaction to something she may be using
on her shin on the nipples. And ready to try anything to get relief for her!
After applying Hot and cold packs then massaging I finally started the flow
and relieved the pressure of full breasts but the Areola seemed to be a real
problem through the day! As there was no Cabbage leaves or ice packs(we have
no Fridge here on the ward) this treatment was followed two three hourly
till relieved enough for comfort the cold washers in a plastic bag being
left on until hot with body temp was doing more than the hot packs! So these
took over. By late in the day the areola was still crackly on touching but I
was able to get bub to suckle that breast alt feeds.
 It was funny next morning though still puffy the areola was comfortable and
the Happy baby was feeding alternating breasts which had now gained
respectable soft proportions and Mum was happy!

Can anyone enlighten me as to 'what it was' and /did I pursue the right
treatment!' Any way, it worked! What to do "next time'
Doctors had never seen it before!

It hasn't snowed here  in Quetta, Pakistan and I fly out to Germany at the
weekend for Christmas with my Hubby's Relatives.
I may get a white Christmas Yet!- my only one ever was in Tasmania and I had
to did the ambulance out for a house call in Mid Summer in Australia.
