Not only do two RNs or LPNs have to check the breast milk before it is given,  they also have to sign the flow sheet that they did this. If breastmilk is not given when it is available and formula is given instead "because the nurse  doesn't want to do the double check ...that would be considered an incident and treated like a drug error in our unit. We are a large Level III (59 beds), Level II (37 beds) Unit.
When we first started the double signing there was a lot of grumbling but now it is just done.
