I have worked with moms like this. Very challenging. I do not use breast shells. Research does not support their use.
This might sound obvious, but has mom tried a nipple shield. If you can, try various sizes.
What I find helps, is to get very creative, first, with the positioning. Mom might have to allow her breasts to fall where they naturally would and get creative with placing the baby there. On the other hand, mom may want to support her breast on a pillow, folded blanket, or even with her sitting on a chair and baby on a bed next to her.
Then what I have done successfully to achieve a latch, is to sort of protrude the nipple by hand, and then sort of pinch it out and actually place it onto the baby's tongue during a latch attempt. One even has to hold it for a moment until the baby draws in the tissue. Moms really need a second person to do this. Over time, many babies can latch on well, if mother continues maintenance of milk supply and strategies to help her nipple draw out.
