While I am really pleased that Dr Morton has formalised this knowledge, I
think there is another cultural difference going on here.  Early hand
expression of colostrum for preterm babies is protocol in most, if not all,
hospitals here (certainly in this state).  Judging by the experiences of the
mothers I have worked with who have premmies, a pump is not usually
considered at all until there are signs of LII (anthough not everyone would
know that it is called that).  Sometimes I think that the staff are so
concerned about the mother's emotional state (or she is unwell due to PE,
Eclampsia or HELPP) that they do not pursue expressing agressively enough
inthe first few days but that is a whole other issue.
