You set your fee based on your experience and expertise, and what the market
will bear. What others charge should not be the major (and certainly not the
only) consideration.

Yes, you want to be within realistic fee range for mothers in your area (an
IBCLC in Manhattan, NY can charge more for a consult than an IBCLC in Fargo,
ND) ... but like any professional (medical, financial, legal) your fee is a
function of your specialized knowledge and the time you spend with the mom,
and in administrative matters (charting, reports to docs) that come
afterwards.

If you are starting to resent the money you are getting for your expertise,
then it is time to re-examine your fee structure.  Sometimes something as
simple as adding a travel fee for those far-away moms will make it seem
"right".   There is a chapter in Linda Smith's Book "The Lactation
Consultant in Private Practice" that discusses factors to consider when
setting fees.
