Hi all,

A few days I posted asking this question. After reading all the evidence I could find and writing and getting answered by some of you out there (thanks again!) this is the consensus that has been reached and proposed at my hospital:

1. HCV seropositive mothers, asymptomatic and with HCV-RNA titers lower than 1 million copies: breastfeeding
EXCEPT if there is nipple trauma with bleeding, in which case the mother will be asked to pump and dump until the nipple trauma is resolved. She will be offered banked human milk, if it is available, to give to her child meanwhile.
  2. HCV seropositive mothers, symptomatic and7or with HCV-RNA titers higher than 1 million copies: will be asked to pump and dump until the acute phase of the hepatitis is resolved. She will be offered banked human milk, if it is available, to give to her child meanwhile.

The 1 million copies was proposed as an extra-safe barrier, since the documented cases of mother-to-child transmission thru breastfeeding I have found had much much higher titers (2.250.000.000). Still, one million beats the 100.000 that was the limit they were using at the hospital.

I hope this might be helpful to any of you interested. I would love feedback if someone does not agree or would like to comment.

  Warmly,

  Kika
