I sometimes charge for in-person follow up and sometimes do not. I always do phone follow up 2-3 days later which is included in my initial fee. If things are going well, that is the end. I have the luxury of being able to do some pro-bono work if I want to. 

I charge an hourly fee for home visits, and I only charge for one hour's fee for follow-up (no matter how long it takes). So my follow up fee is lower. I will come back for free when I see that this is a serious situation that one visit won't cure and money may be tight for the family. 

I do charge for those moms who just want me to come back to have some support, or just someone to listen to them. 

My biggest problem is those moms who call me over and over about little stuff after I have seen them once or twice and the problem is cured. One could spend hours a week taking free phone helping calls. (I do give referrals to LLL for this type of phone help.)
