Cheryl, I did not mean to imply that insulin resistance is *a* known
cause of poor milk supply in and of itself. In trying to be succinct, I
did not elaborate on the potential connection. Within PCOS, insulin
resistance is very common. New insulin-sensitizing drugs are being used
on women with PCOS resulting in improvements in multiple hormonal
imbalances beyond insulin itself. Mona Gabbay MD recently presented some
cases to the Academy of Breastfeeding Medicine on the use of one of
these drugs for improving lactation. While many of the women had PCOS,
the title of her presentation specified insulin resistance rather than
PCOS--- that was her focus. Certainly there are many women with insulin
resistance who lactate just fine, but there is also a subset of women
who have experienced increase in supply when treated with the same
sensitizing medication. Chances are that a cascade effect is occurring
in reverse as an indirect link to lactation. However it works, when I
hear "obese" and "supply problems" I think of PCOS and insulin
resistance and the correlations noted to date, and realize that there is
more below the surface than meets the eye.
