Regarding the mother with delayed letdown perhaps fennel tea before pumping 
 and in this heat she could make a large pot once a day and refrigerate so 
she  has enough for several cups, Rescue Remedy, 20 min before, then 10, 
then during  pumping another dose of several drops, reverse pressure softening 
before and  again after on pump for few minutes, and has she tried pumping 
one breast and  nursing the other.
I imagine she's quite busy with five other children and distracted so  
putting older ones in charge if possible while she pumps in another room might  
also help.  Just some ideas she could consider.
