RE: Arly"s question about how to address doctors. In my book, this depends
on one's relationship with him/her/them.

For some MDs I know well, we are on a first name basis.
For others, I use their title for them and they use mine for me.
When someone I do not know calls me by my first name, I reply the same way
and this usually stops them for continuing to assume they know me that well
unless I indicate this is ok.  (I have a personal bugaboo about seeing an
MD as a patient and being addressed by first name when I have not suggested
this first.)
