I am seeing clients at their homes in a private practice setting right now.  I do fill out their 
paperwork with dates, names, brief hx before I go there and that doesn't take that long.  
For me, a home visit usually lasts about 1 1/2 hrs-sometimes closer to 2 if I watch them 
pump as well.  These are usually involving pre/post feed weights as well.  I'm fairly slow 
when I write my MD reports but I do make them a little more involved because I want to 
pass on the information since I don't otherwise have an opportunity to talk with the 
physician.  

45 minutes would be a pretty short time but even my shortest appts are about 1hr 15min.

Hope that helps.  I think the shorter the appts get the more the mother may feel that the LC 
latched the baby and she didn't have an opportunity to try latching comfortably herself or 
didn't get to see what real swallowing looks like or what the baby looks like at the end of a 
successful feed.
