I think I was more responding to the folks that said that "jumping" to
formula at 48 hours was inappropriate in a healthy baby.  Unfortunately, I've
seen too many moms that have had major birth interventions be unable to hand
express any colostrum for quite awhile, certainly not the 7 to 14 ml that the
"normal" baby would be getting when breastfeeding during the first 24 hours.

For a tongue sucking baby, I'd try fingerfeeding using a tube on the breast
to teach him/her how to suck with something bigger in his mouth than a
tongue.  I'd start w/ mom's pinkie, then middle finger, then thumb, and
progress to daddy's thumb which is generally larger -- attempting at each
feed to get punkin on breast, but not so long each time as to frustrate both
mom & baby.  I've usually found, IME, that it takes about 3 days of being
persistent w/ this for babies to get on the breast comfortably.
