Hello again,
Sorry for the search post... at least you know I did search the archives
prior to posting... but apparently can't read the new addresses properly!
sorry.
Here's the story behind the search: Baby boy, mom's third, BW 7#7oz, up to
7#8oz at discharge, down to 7#7oz at 14 days. Began supplementing with 20CC
formula in SNS during every bf.(Mom hx of "no milk" with previous 2
babies.. tried SNS with at least one and had little or no milk, no history
of illness, hemorrage, damage etc reports all her babies have weak/sleepy
suck habit, had epidural 1 hour and stadol).
Baby making at least 4 sometimes 6 or more wet cloth diapers/day. But on
Day 18 baby was dry for 3.5 hours. Gave 60CC by SNS baby started wetting
and had a BM, immediately. Mom got good hosp. pump and pumps 40-55CC per
pumping after each feed, is using that in SNS with formula to top to 60CC
per feed. Day 22 baby up to 7#14oz, day 24 8#3oz.
Baby sucks vigorously on finger, often falls off large soft nipples, but
will suck there, not very strongly. Sucks vigorously at breast w/ SNS
attached. Occassionally mom reports she feels "draw" of milk through breast
up into armpit (let down)and leaking from other breast, but not w/ every
feeding.
Here's the question, Mom tells me her LC says that if she can't pump 60CC
at every pumping (this is after feeding at breast w/SNS) by the end of this
week that she'll never be able to make enough milk to feed this baby. That
the baby needs 20 ounces per day. (Mom too anxious to do a pre/post feed
weigh, ?are those very telling?)
        MY questions:
? quantities pumped do not necessarily reflect amounts produced, correct?
? Is there a table of how many ounces a baby needs of bm by weight? does
20/day sound right for an 8lb baby?
? Is there some defining point at which a woman's body simply can never
make above a certain number of ounces? (hinting at "insufficient glandular
tissue")
? wouldn't metaclopramide be of use
? if the baby is gaining 2oz per day on this 60CC mix of pumped
milk/formula in SNS at each feed, couldn't they try only the pumped milk at
the 40-55CC range and see how he gains?
? is the 1/2 ounce to 1 ounce per day "rule" still an accepted guideline?
? anyone got a great tip for getting this kid to suck vigorously at
breast? semi upright football hold seemed to help some, but not enough,
apparently.

Thanks for being here!
aLLLways,
anne
