Hi, all, another question:  I have a client, infant 3 weeks old, good
weight gain, good milk supply, exclusively breastfed with rare bottle feed
of pumped milk by dad (maybe 3X/week),  mom is having problem with a
sensitive irritated feeling on bottom of nipple/areolar junction on one
side only.  There is nothing impressive  to see on the nipple. So far, I
have helped with positioning, deep latch technique, helping this little
girl try opening her mouth wider, making sure her bottom lip is flanged out
(she sometimes will try to suck it in), and mom has treated for thrush with
GV, only because after a week of diligent positioning she still had pain
and wanted to try it.  She has tried APNO also, before I came into the
picture.  She describes the feeling like a rough cat tongue each time the
baby's jaw moves during a suckle.  Yet the baby's tongue is extended well
over the gumline and she is not thrusting.  She says it feels this way in
cross cradle, football, and side-lying, which is interesting since the
tongue is positioned differently on the area with each nursing position.

  I am running out of ideas and when I see her again today I am thinking I
might try a close contact nipple shield to see if she still feels that
irritation.
Any other thoughts for me?
