In this very small island of 1.5 million people, 60 by 30 miles big and
with about 22,000 births a year, centralized hospital facilities and very
good transportation services, SIDS is almost unheard of (I have never had a
case in 20 years of practice at the public hospital and in private
practice).  Except for a certain "socioeconomic class", most babies sleep
with their moms and breastfeed at lib throughout the night for the first
