Diabetics (as the mother in the original post was, no?) are well aware of
the signs and symptoms of ketosis, the condition which occurs when the body
is deprived of carbohydrates and has to burn stored energy to fuel itself.
One sign is a characteristically fruity-smelling breath.  Maybe this mother
really is describing something abnormally sweet-smelling.  Most likely not,
but no one would know without checking the baby.  If the baby is ketotic, it
is starving and her concern is more than justified.
Not trying to be pessimistic, just trying to listen to the mother
