Do you feel certain that it's a baby issue vs. a mother issue?

A mom who'd been in our LLL multiples group presented with a similar
situation when she had a singleton about six years after BF her twins. Baby
appeared to BF/transferring milk well and ate on cue 9-10 times per 24
hours, plus mom pumped after BF when able because baby wasn't gaining at 2-3
weeks. Like this baby, mom was reporting 4 or more stools/ 24 hours and
plenty of wets. However, the stool I saw when there was very small --
although it was more than a skidmark. When I questioned mom, I found that
baby rarely had a larger, more profuse stool.

Mom had a history of PCOS and had taken metformin during pregnancy, which
the doc stopped at delivery. After my visit the mom asked the doc if she
could go back on metformin. He was reluctant but said OK to a trial. Within
days of restarting on the med, milk production must've changed because baby
started gaining -- and gaining well! BTW, this mom had required nothing
extra to produce adequately for her twins years earlier... apparently the
PCOS escalated.

I've also seen moms who turned out to have some thyroid issue whose babies
presented with similar BF "stories."

Good luck with this mom-baby...
