I would have to say in my sample of one that onset of lactogenesis II in <24
hrs is more normal for a home/homelike birth.  I would tend to think that it
is more a result of an individual women's own hormone makeup.  Just like
some women have a return of fertility much earlier/later than others who
breastfeed more or less in the same type pattern.  My last two babies (which
were my 4th and 5th) were definitely home-like.  We went home 3 hours after
birth with both of them because they were born on the evening shift and the
hospital told us their insurance didn't allow the babies to stay in mom's
room overnight!!!  Even though both of our docs were willing to release us!
My total hospital stay was 5 hrs with my last child who came 1 1/2 hrs after
admission.  We came and left on the same shift, the staff just couldn't get
over it.  Still, my milk came in on the 3rd day with both of them.  I also
had lochia much longer than most women I knew (I always made my pp check-up
for 8 wks, bc it took that long for the bleeding to stop!)  I know that L-II
can be delayed by birth circumstances, but I wouldn't go so far as to say
that it would come in earlier than 24 hrs if most women had a more
physiologic birth experience.  But who knows, could be a lot of factors that
contribute to what we consider normal that might be different under
different conditions.
