Janet,
 Several weeks ago I started having the same thought. In routine postpartum
histories I ask if the mother is experiencing MER. More often than not they
aren't aware of MER, but visual assessment of a feeding gives evidence that
it is occuring. Am I reading your question correctly? That the woman isn't
aware of the MER; or are you saying that it doesn't occur?
