Sounds like your sweet baby is doing the best he can to maintain his airway 
 while breastfeeding with an over-active milk flow/amount. When feeding on 
one  side does the other breast squirt or dribble milk? Are you double 
pumping?
 
Baby chomping and clamping can be to stimulate flow or, in this case, seems 
 more likely to be done to *obstruct* milk flow so that he can breath. If 
he were  to open wider he would not be able to control the flow as well. 
Smart baby!  Milk from fast overabundant flow and amount may be the cause of wet 
breathing  sounds, gulping, coming off (to catch his breath), choking (as 
you say with or  without letdown), also milk may be backing up into nasal 
passages, possibly  causing some irritation/edema. And/Or he may also have some 
reflux, ...too much  foremilk per feeding?
 
Suggestions to try:
 
-pump off excess milk before feeding
-you say you tried "elevated" position, was that with you sitting very  
upright? ...was baby as upright as you could manage? ...try you  supine/on your 
back, with baby laying on top to feed, express drops so baby  knows it 
works this way, keep trying this position esp when baby is not  overly hungry 
and can tolerate practicing.
-consider decreasing or suspending milk donation for a while, at least not  
double pumping. 
-if all else fails, consider using a shield (Yiks!) till nipples are  
healed and flow/amount is manageable by baby
