Oh Crystal!  So sorry to hear of the loss of this mom.  You didn't miss
anything.  Pulmonary embolus is a complication of birth; it's very rare - I think
<1% of all births statistically.  But, to put it into basic terms, after birth
a clot forms and travels to the lungs.  Even when immediate interventions are
instituted, this is a critical condition.  Some women develop pulmonary emboli
while still in the hospital setting after birth, and they get whisked away to
ICU and receive lots of blood products such as platelets or fresh frozen
plasma to facilitate the clotting factor.  Also, receive clot resolving
medications such as heparin via IV administration.  Just as you described, these women
can be absolutely fine one moment, and then the next second gone.

I feel for your loss...I would be devastated if one of my clients died.  I
can't imagine how you must feel!  But, know that you did not miss a thing!  Do
something special for yourself this weekend....take a walk, get a massage, get
your nails done, talk to a friend....hug yourself.
