I like the idea of asking about infant feeding on standard health hx
forms. I always ask it when I do my research, even if the info isn't
directly related to the research I'm doing because I think it should be
part of our awareness.
I suspect it will be a while before it can be used for research
purposes with any reliability or accuracy, but in the meantime it puts
the importance of bfing on the radar screen of hcps and consumers.

Where do these forms come from? CDC? medical organizations? are they
commercially produced?
