Dear Lactnet Friends:

A mother I worked with had nipple too large for her baby's mouth. She was
given a nipple shield in the hospital. It hurt her, and was a barrier to
breastfeeding, even though her baby would breastfeed with the shield in
place. However, she stopped breastfeeding due to pain.

I've had some lovely private emails from Lactnet Friends (thank you all!)
describing how a nipple shield can make latch easier for a baby with a
little mouth, and it still makes no sense to me.

If the mouth is little, grasping something too big is not possible. Can't
get the big foot in a little shoe, even if you put a sock on the foot.

If the mother has a large nipple, then one has to use a large nipple shield
for her comfort...but the baby's mouth stays the same size.

Please, Lactnet Friends, I understand that there are many clinical
situations where a nipple shield is helpful. The oroboobular disproportion
is not one of them. She'll have to hand express or pump until her baby
grows.
