I once helped a mother whose baby seemed to cry in pain at the breast as you
describe. We finally figured out that the baby had a bladder infection -
when the baby drank (from breast or bottle), the baby would pee and this
would cause the pain and crying. A pacifier didn't seem to stimulate that
peeing reflex. So I would have this checked out.

I know that ear infections or fluid behind the eardrums from an old
infection can cause this kind of crying when the baby sucks and swallows,
too.
