I have an outpatient appt form that I give to parents. It tells all

about the appt what we do where to go etc.

We do not have a consent form.

We have our own chart which says 'not a part of the medical record.' I

file and keep for 1 yr.

I will send you an attachment, Amy, of my outpt form.

To me, it implies consent.
