I don't, but I've put out a request to our hospital's LC to see if she
does--I'll be seeing her on Friday.  If there's something you can refer
me to in the meantime, I'd be grateful.

Another issue regarding his difficulties is that it seems to be hard to
get him latched on deeply because of the nipple shields (Medela).  They
seem to provide a "stop" to his forward progress.  KWIM?  He can only go
so deep.

Hi Beth:

1) There are directions for using "dancer's hand" technique in a booklet by Sarah Danner, one of several booklets she wrote in tghe 1980s about special situations in breastfeeding. I don't know if it is currently in print but I hope it is.  There is also a photo of "dancer's hand" technique in most editions of my first book, Successful Breastfeeding [theAustralian one], but that has been out of print for years.  (Can someone else help Beth with where to fnd a picture or diagram?)

2) I wonder if the Medela nipple shield your client is using is a good fit, 'cos different breasts may need different brands for a good fit.  One of my clients found she could get more of the breast tissue drawn into the nipple part of the shield by warming and stretching it with her two thumbs, i.e. widening the base of the nipple part.  The brand she used was a different one, but the idea should work with other brands.  Just some ideas.... not sure if they'll help. Maybe try a couple of different brands? - or see if the little baby will latch without (if you don't try when he's very hungry), at least some of the time.
