I am consulting with a mother with 2nd child, now 14 days old.  
First child BF for 18mo, now 3 yo with no issues.  
Initially consulted as mother had severe engorgement on day 6 - 1 day - post discharge & with bleeding nipples, skin missing from both nipples, milk slow coming in as 'c' section & meds 
Taught hand expression and finger fed baby - as nipples extremely painful.
Mother asked me to come to see her as nipple are "D" shaped when baby off the breast - 
2nd consult on day 11
have tried various positions - found football/underarm hold to be the least painful - however nipples are still distorted.
any suggestions would be most helpful.
Will see mother again this week.
