Haven't seen this myself at breast but when bottle fed babies do this gentle
upward pressure on the jaw helps.
I would also suggest that mom try the *dancer hold* rotating the hand that
supports the breast so that the baby's chin rests on the fleashy part between
the thumb and index finger.
Any chance the baby is tongue thrusting and pushing the milk out of the
mouth?
