With bilateral mastitis, I would suspect strep infection. Someone else
mentioned MRSA infection. The baby's nose and throat could be cultured too.
I am not sure what antibiotic would be appropriate, but perhaps a consult to
infectious disease would pinpoint this, and the length of time she should be
on the antibiotics. I agree that 3 hrly feeds is quite infrequent. More
often would help her supply and draining of the breast. I am not opposed to
the use of a shield, as a temporary measure, if it helps baby to latch and
gets the milk out.
Hand expression, gently, might work better than a pump. Mom should be on
bedrest with baby and getting good nutrition and hydration. I would advise
her to take a probiotic to help ward off yeast.
