I am taking this from my notes from a conference in Hershey, PN last year.
I can't do it justice but it was interesting so I'll try to share.  A
medline search tonight did not show anything published yet.

Paula spoke about using nipple shields for premature babies whose mothers
had a good milk supply to facilitate transfer of milk.  She said the shields
allowed the infant to keep the nipple elongated better by maintaining a
higher intraoral suction and this allowed better compression of the sinuses.
She said this increased the measured volume of milk transfer for the babies
she researched.   When asked why she thought this worked theoretically
because it allows the baby to use expression without using a lot of suction
to transfer milk.  There is a high negative pressures in shield between
nipple end and shield end and the baby can take milk with primary use of
expression, not suction.

If anyone knows if this has been published yet please share the citation.
If not, I hope and expect it will be sometime soon.
