My first question is -Was this mom an overproducer for the full 15 months?

I am concerned that her oversupply has not lessened significantly in three
weeks of focused effort. If it is new onset it seems even more unusual.

One of my partners had a client last year who was a huge overproducer for
her twins. It seemed like such good news, but it never went down. Then she
began having terrible migraine-like headaches (was hospitalized for them).
Finally she was diagnosed with a pituitary tumor, which unfortunately,
explained it all.

Please continue all you are doing and also have her evaluated by her primary
care provider if needed. In retrospect with the case above we waited too
long by thinking it was a bf management issue.
