I would think the superbill from California would give you an up-to-date
list of appropriate numbers. The number I have is (818)841-4182.
I usually use these CPT's - 99245 for an extensive consult
                            992125 for about an hour f/u

Common diagnoses include:  FTT 783.4
                           Feeding problem, newborn 779.3
                           Feeding problem, infant  783.3
There are lots more!
