Dear Claire,

Of course I'd counsel the parents on other dietary measures. Since this is
an educated, breastfeeding, vegetarian family, I assumed that the child is
eating a relatively healthy diet durng the daytime. If he was my patient,
I'd get details of what the child eats and drinks during the daytime and
steer them towards healthier, more nutrient dense foods and away from empty
calories. Susan mentioned in the post that the child is physically active.
In a child that is growing so well, hypothyroidism is unlikely. If his
development is normal otherwise, genetic syndromes that present as obesity
are unlikely as well.
I maintain my opinion that drinking breastmilk every 1-2 hrs at night at 2
1/2 yrs of age has very little physiologic benefit. On the same line, night-
time weaning is not going to cause psychological damage as long as the baby
is well attached and loved in other ways. Depending on how much milk he is
consuming during the night, he is probably getting 200 to 600 calories
extra every day. This toddler would do better to drink the same milk during
the course of his day. Nighttime feeds at this age are mostly for comfort
and not physiologically necessary like in a younger child. Just my 2cents
worth.
