Yesterday I discharged a 3+ day old baby who was
"improving" with BF. I offered to mom they could stay
but I understood she was getting "Hospitalitis" Anyway baby
was 91% of BW after dropping to 90% at 72 hours and was getting D5W suppliment post feeds. Mom or dad was
"finger feeding" I sent them home around 10 AM with
instruction to call me at 6PM.
Mom called upset baby had not latched since leaving the
hospital and now was sleepy wouldnt try to feed. She had
give this 3.5kg (BW) bay fromula 20cc 1 hour prior and also
said the house was hot. I told her try again in 1 hour and I'd
call her. Baby did better so we discussed acute options and
last night she attemped BF used hand pump and finger fed.
I saw them this AM. Weight up 30 grams, urine X 5, no stool.
The catch is Im alone without a peds nurse.
By myself I had to get this baby to latch.
His problem seemed that mom's milk isnt "in yet" baby latches
sucks alittle then seems to get fustrated and fusses and
comes off.
I finger fed him some junk food (abm) to calm his hunger
alittle then reattempted latch. (Parents were finger feeding
with a syringe so I showed them with just a finger). Alittle better
but same result.
I then taped the feeding tube to mom's breast and baby fed
better. We even tried the second breast w/out the tube and
baby sucked for a few minutes.
This should work at home as dad is an ER nurse and nurses
tape bettter than docs.
I then had dad give an ad lib finger feed and let them watch
the Medela/ Chele Marmet video. Mom has found a battery
pump at home but I recommended an electric and Fenugreek
and B thistle.
