Now that I look at your questions again, I don't really have answers for
either of them.  There might be useful information for you on the HMBANA (Human
Milk Banking Association of North America) website that I believe is hmbana.org.

I am not personally sure how much demand there is for pasteurized, banked
milk.  It seems to be the rare physician who is aware that it even exists and is
available.  Since a prescripton is required, this can be an obstacle (as well
as $ and other issues) even for the mom who is extremely dedicated to
providing human milk for her baby.

I know that the Denver MMB has a newsletter (and I imagine the others do,
too) that describes some of the recipients and how they have benefited from
receiving donor milk.  Their number is 303-869-1888 if you wish to contact them.
