Laurie writes of frozen breastmilk gone bad....

There is a bit on this in the Breastfeeding Answer Book regarding some mothers who have milk high in lipase, an enzyme that breaks down fat in milk. Scalding the milk before freezing inactivates the lipase. Some mothers have this with all cooled milk, other with only frozen milk. Sounds like this may be the issue with the mother you are working with?
