Dear Folks:
  I am working with a woman who came to me when her baby was a month old
because the pain wasn't going away. The tips of her nipples were about 1/8
eroded away. OUCH.  :-(  No wonder.
         Baby has great gape; mom could not get him on when he opened wide as she
would back off from the latch a split second before contact because of the
nipple damage and pain. So the latch was shallow and there was a lot of nipple
compression. No signs of oversupply; she uses one breast per feed and the baby
has been thriving all along. She is now healing (rested nipples with pumping
and bottle-feeding,  using Second Skin as a dressing when breasts in bra,
stopping the  use of breast shells.  Someone in the hospital told her she had
flat nipples and had to use shells. She wore them non-stop for a month, except
when breastfeeding!) She is progressing nicely, and has terrific help from her
husband, who came with her to the visit. Okay, now for the problem.
        She reports burning/tingling  breast pain after a feed that lasts for about
20 minutes. It is very uncomfortable. These episodes have been occuring in the
evening, only once or twice in 24 hours. She's noticed this for several weeks.
        I have suggested warm compresses and/or massage, thinking that it might be
duct cramping. Seems to me that if it was thrush or some infection, it would
hurt all the time and be getting worse. It isn't. She has had a day every once
in a while when it doesn't happen at all. Any ideas? Thanks, Nikki Lee
