Hello, I need to start by saying how proud I was to be able to tell this mom
that although I had no prior experience with this situation, I could ask
hundreds of colleagues on the 'net and get back to her. Thanks in advance!!

This woman's fraternal (girl) twins were born 2/21/97. Their weights were L:
7lbs 9oz and A: 7lbs 3oz. They were 37 5/7 weeks gestation at birth. (14
weeks of bedrest-terbutaline pump, MgSO4 and p.o. terbutaline at various
points throughout pregnancy) The mom had contacted me prenatally to discuss
breastfeeding twins, at the hospital, she called to say they were doing
great, and by the time they got home, she called to schedule a consult for
cracked nipples and general concern about nursing them correctly. At 5 days
of age they weighed L: 6lbs 14oz A: 6lbs 8oz. Basic corrections in
positioning and latching were all that was needed at the time. 3 days later
another consult was requested by mom to to check that they were continuing to
do well. Weight check was deferred at mom's request, both girls were nursing
well with adequate wet diapers and stools. She reported that both girls were
vomiting several times per day, that it was sporadic and didn't seem to have
any specific relation to anything she could identify. They had been seen by
the pediatrician that morning and no cause was apparent. Approx. 1 week
later mom called to say that both girls had been nursing the same, had
discontinued the vomiting, but had very distended abdomens and had not gained
any weight in 1 week. The pediatrician ordered an abdominal ultrasound for
one of the twins, (she was much more distended than her sister) and it was
negative. 2 days later stool samples were obtained to check for 1) allergies
2) infections 3) ability to digest and absorb complex carbohydrates. While
the first two were negative, both girls came back 4 plus on the absorption
test, and the ped is considering switching the babies to Nutramigen.
Needless to say, the mom is very upset by this. The real mystery is that in
the 2 weeks since the sample was obtained and the results available, the
girls have had no distension, and have both gained an average of 8 to 10 oz
in each of the past 2 weeks. The pediatrician is also confounded, and is
checking with a specialist. Anyone have experience with this? Could they
have grown out of whatever the problem was? If anyone has any references I
could pass along to the ped, that would also be greatly appreciated.
Again, thanks in advance.
