There's a decision rule in this article:

Anjana Srinivasan, Carole Dobrich, Howard Mitnick, Perle Feldman.

The only change I'd make is for the requirement of a visible frenulum. 
Frenula can be submucosal and invisible unless the tongue is elevated...
