Hi Liz,

 

Here are my thoughts after letting this run around my head for a day or so.


 

The breast, when baby is latched deeply, fills up the mouth going back to
the hard/soft palate.  I am thinking this is NOT a nose thing - but possibly
it is an airway constriction thing due to where the breast is being placed
during BF.  My thoughts are:  maybe an overly large Uvula that gets pushed
back into the airway during BF.???  Something along those lines - some
anatomy 'thing' that gets pushed back as the breast enters deep into the
mouth, but occludes the airway just enough to cause a problem - possibly
even sucking in some milk into the bottom of the nasal passages giving that
'stuffy' sound - but not high enough to get into the passages and have
anyone actually get milk from the nose through bulb syringe.  (Large
adenoids or tonsils??)  What about positions?  Has mom tried different
positions (like baby sitting straight upright) with any luck?

 

A bottle nipple doesn't go in as far or fill up the mouth like the breast,
which would make sense to me why he has no issue and why no one can suck
anything out of his nose.  Have you tried doing a suck evaluation with a
flow of milk (p-syringe) to see what happens there?  If our finger goes back
to that juncture of the soft/hard palate it may be possible to somewhat
recreate the issue baby is having on breast.

 

Just my thoughts - let us know what happens.  At the very least - to keep
baby comfortable and happy during feeds I would probably discuss with the
mom the option of pumping and bottle feeding.  Taking 25 minutes to get 1.8
oz and all of that being very stressed says to me a set up is happening for
baby to flat out refuse to BF soon.  I would keep feeds happy - lots of Skin
to Skin.
