Mandi,
       What a difficult case you have described!  And how frustrating for
this poor mother!  It does sound as though Mathew may be doing all that wiggling
to distract himself from some kind of pain.  I don't know if this will help,
but I have worked with three babies who did not stool frequently, and mothers
had to "move heaven and earth" in order to get weight on these babies.  In all
cases, babies were meeting developmental milestones, so docs were not
concerned, but mothers were exhausted.
       In each of these cases, it turned out that a vertebra had twisted and
was riding on the nerve that controls the intestines, so the intestines were
not emptying well.  That meant that baby never had a lot of room for large
feedings, and was often bloated and uncomfortable.  In all of these cases, a
pediatric chiropractor was able to help.  This situation is not a quick fix.  It
took several weeks of twice-a-week visits, but at the end of that time, all the
babies were stooling easily and frequently, and they were also eating more
normally and happier all around.  You mentioned that Mathew eats well at night.
Does he perhaps stool in the evening, so that there would be more room for
larger feeds at night?
       Just casting out ideas.
