Gonneke -- thanks for the question.  I will clarify my earlier suggestions
re: consents.

Yes:  ANY consults -- by mail, e-mail, phone, Skype, or in-person, should be
conducted using the same formalities.  We all get a little loosey-goosey on
the rules, especially when we get that phone call from the mom with "just
one question ...."  But in the most professionally-run world, we IBCLCs
would be documenting all of our contacts and getting paid to boot.

Ask a doctor or midwife the last time they spent 20 minutes chatting on a
phone with a mom, and did not chart somewhere.

You are absolutely correct that a mother who contacts you first has given
pretty darned good "implied consent" to be seen and advised by you as an
IBCLC.  As I like to say:  there aren't a whole lot of door-to-door IBCLCs.
What I am suggesting is that it is wise to note in your charts however it is
that you and the mother came to a "meeting of the minds" that she avail
herself of your services.

I did loosely use the term "consent form" in my post -- sorry about that.
My consent form covers the mothers consent to be seen -- but all the other
stuff too, like:  her understanding that this means I will communiate with
her other health care providers .... the possibility that I will touch her
breasts/her baby/her baby's mouth ... consent to be photographed for
educational purposes only (though I never do carry a camera) ...
USA-specific privacy language [HIPAA] .. and -- not unimportantly -- my
expectation that services be paid when rendered.
