in our school, not only did you need a doctor note, you had to take the
stupid milk anyway and dump it after because school regulations required
the giving of milk even if a child would not drink it.  appalling.  Even if
one is not anti milk (I'm not anti cow milk just anti YOU MUST DRINK
THIS!!) the waste factor there is disgusting.

oh and NO substitute was offered or given. you had to bring your own little
juice box or whatever.  this was in the 80s in Nebraska (can you say
"mooooooooo...."?)
