Hi. In my 99 Hale, Decadron is on p. 193.  "Dexamethasone- Not reviewed by
AAP, Dexamethasone  is a long acting corticosteroid, similar in effect to
prednisone, although more potent.  Dexamethasone 0.75 mg is equivalent to a
5 mg dose or prednisone.  While the elimination half life is brief, only 3-6
hours in adults, its metabolic effects last for up to 72 hours.  No data are
available on the transfer of dexamethasone into human milk. It is likely
similar to that of prednisone which is extremely low. Doses of prednisone as
highas 120mg mail to produce clinically relevant milk levels. This product
is commonly used in pediatrics for treating immune syndromes such as
arthritis, and particularly acute onset asthma or other bronchoconstrictgive
diseases.  It is not likely that the amount in milk would produce clinical
side effects unless used in high doses over prolonged periods. Adult
Concerns: In pediatrics: shortened stature, GI bleeding, GI ulceration,
edema, osteoporosis, glaucoma, and other symptoms of hyperadrenalism. No
pediatric concerns reported via milk. Drug Interactions: Barbiturates,
phenytoin, rifampin may reudce the corticosteroid effect of dexamethasone.
Dexamethasone may decrease effects of vaccines, salicylates, and toxoids."

So, the problem is probably a non problem for this mother, and the burden or
responsibility lies with the practitioner who recommended against
breastfeeding. I would politely request a copy of the documentation (not the
PDR) that supports the recommendation to temporarily wean.

I would recommend that anyone who does not have an updated copy of Thomas
Hale's book buy one now....it is an invaluable resource.
