HI! My name is Kirsten Supancheck and have just joined lactnet. I've had
a delightful time reading all your great messages this last week. I am an
RN and IBCLC working in a large hospital in So. California with another
RN/IBCLC. Our current delivery rate is about 500 babies per month. We
have a level 3 nursery with anyone 25 weeks and up. Our
breastfeeding(pumping) start rate in the ICN/NICU is about 90% for the
preemies and we are currently seeing them post-discharge in small groups
each week. About 80-90% are still pumping at discharge and starting to
feed at breast every feed. Hence my problem:

One of the neonatologists is extremely supportive of breastfeeding and
has consistently pushed for increased breastfeeding in these babies.
These babies go home starting at about 4 pounds and 34 weeks up. We are
writing discharge instructions about feeding the breastfed baby at home.
She would like to say the following: "Your baby will eat about every 3
hours. Some babies may eat as frequently as 1 1/2 - 2 hours. Occasionally
you baby may sleep for 5 hours. If your baby sleeps longer than 5 hours
wake him up for a feed. Otherwise let the baby determine the time for the
feed." Her belief (as I understand it) is that the preemie baby can go on
to demand feed at discharge.  Have any of you any experience with demand
feeding the preemie? The babies are doing well on the current 3 hour
structure. I  also use a breast/bottle/pump combination every feed until
I can document adequate intake. What is your "community experience"? This
instruction sheet will go out with the HMO letterhead and triple NCR so
it will be placed in the hospital chart.
