Agree.  And when mother is using "upright feeding position" does that mean mother is upright or baby is upright?  I have found biological feeding with baby on abdomen much better for "reflux" babies.  Remember when we used to put babies on their bellies after feedings with knees tucked under?

I'll let Cathy tackle what surely sounds like a tongue-tie problem. (And by the way, the ability to stick the tongue out is not indicative of good
function-- they must be able to extend tongue while mouth is open wide AND keep it out). As for the milk supply, mom needs to understand that she has a baby who struggles to stay on the breast, struggles with feedings, and she is intentionally inducing a double supply, which essentially creates a higher pressure MER. Now if she had twins, that's just the way it would have to be. But since this is a singleton baby, producing half as much milk just might reduce the back pressure and the strength of the let-down, which in turn just might make feeding easier and require less suctions breaks to control milk flow.  Help mom to see the experience through baby's eyes. She can do some insurance pumping, but it needs to be more tempered.
