In my discussions with the Arizona version of Medicaid, they insist that
only the person delivering the service can bill for that service. No
piggy-backing allowed. When you use a code that indicates a physician
office visit, regardless of the subject / topic of that visit, they want
ONLY the physician to bill deliver and bill for those services. Even if
it's lactation related. They say a Nurse Visit is exactly that and nothing
else and they threaten dire consequences if you include another
practitioner's services in that Nurse Visit. It is just about necessary to
have codes written specifically for LCs.

How do we get new billing codes written?
Quit laughing!!!!  I'm serious!!  We need our own billing codes.  :-/

We also need an official policy from some respected group (the ABM?)
evidence-based, stating that ANY baby in Level II or Level III requires
it's own mother's milk - a medical necessity by definition unless the
mother refuses.
