There may be a number of reasons behind this mother's plan, several
of which have already been mentioned-hx of sexual abuse, fear of
"exposing" herself, etc.  I think the problem is in part due to the
fact that there has been lots of good publicity on the benefits of
breastmilk for babies, but without enough emphasis on
breastFEEDING.  Moms seem to think that pumping and bottling somehow
is "the best of both worlds" even though most of us would see it as
double the effort.
I would certainly start as many others have mentioned, by assuring
her you will help her meet her goals.  I might then suggest that it
might be easier to get her supply well established if she did at
least some "direct" nursing at the beginning.  Baby is the most
efficient pump, baby's mouth is the most effective stimulus to the
nerve endings in the nipple, etc.  Whatever her hesitation about
nursing directly, she might feel she can do it at least at the
start.  Hopefully, baby will catch on and mom will be "hooked" (or
unhooked as the case may be).  If she is not open to this approach,
she may reveal more about her reasons as she states why she doesn't
want to use this approach.
I keep reminding myself that while we would rather see moms nursing
directly, most of these moms who want to pump and bottle would
probably not be giving any breastmilk if they didn't have this
option.  It may not be the best situation in our opinion, but it is
certainly better than formula feeding!
